Upcoming Research Projects
HUTEPH is preparing to launch several forward-looking projects that expand our impact in health equity, technology, and community engagement.
In collaboration with Schneider Regional Medical Center, Howard University will conduct a pharmacist-led transitions of care project. Briefly, the pharmacist will conduct medication therapy management visits within 14 days of discharge to support the patient after discharge. The primary study outcome will be readmission within 30 days of hospitalization. The project will leverage the HUTEPH Centers' SeeUrPharmacist application to conduct the visits and document MTM services.
Howard University, in collaboration with St. Thomas East End Medical Center Corporation (STEEMCC), will conduct a telebehavioral health implementation project. Briefly, the project will examine processes and barriers to implementing care in a collaborative practice model that will include a psychiatrist, a clinical psychologist, and a pharmacist. The project will leverage a new EHR implementation with telehealth built in and the Howard University SeeUrPharmacist application to conduct MTM during the virtual behavioral health visits.
