What We Offer

Telepharmacy Services

Our telepharmacy platform allows patients to connect with licensed pharmacists from the comfort of their homes.Through virtual consultations, we provide medication therapy management, prescription refills, and personalized counseling to ensure optimal medication use and adherence.

Population Health Initiatives

We are committed to addressing health disparities and improving health outcomes on a larger scale. Our center conducts research and implements community-based programs focusing on preventive care, chronic disease management, and health education.

Collaborative Care

Real impact starts with real stories. Hear directly from our students about how our programs have shaped their journey, equipping them with the skills and vision to transform healthcare and serve their communities.

As a P2 Pharmacy Student at Howard University College of Pharmacy with a strong interest in community pharmacy and public health, I wasn’t sure where to begin—but PHIT4DC was the perfect starting point. With my background in serving diverse patient populations, I’ve always been passionate about addressing health disparities at the local level.

This program introduced me to the core concepts of public health informatics, health IT, and population health in a meaningful and digestible way. I've gained valuable knowledge about data standards, value-based care, and the role of EHR and HIE systems in advancing health equity, especially in underserved communities. Most importantly, I’m leaving with the confidence and curiosity to continue learning and applying this knowledge in the community pharmacy setting. I’m incredibly grateful for the guidance and support we’ve received throughout this journey.
